Please use this form to request an appointment with us. We will do our best to accommodate your first requested day and time, but we request that you include a 2nd and 3rd choice of day and time.  Unfortunately, we are not accepting new clients at this time.

There is a deposit fee for all surgical and dental procedures:

• $100.00 deposit for one pet
• $150.00 deposit for 2 pets or more
